However, Delta Mariner owner Foss Maritime’s lack of proper safety oversight and the Kentucky Transportation Cabinet’s inadequate bridge navigational lighting maintenance were also contributing factors.
On the evening of Jan. 26, 2012, the Delta Mariner collided with the bridge, which carries U.S. 68/Ky. 80 over Kentucky Lake at the Trigg/Marshall County line.
A meeting last Tuesday afternoon in Washington, D.C., featured a full report from investigators, after which the NTSB handed out 14 findings and five recommendations.
The findings mention the navigational lighting on the bridge, the failure of the crew and contract pilot to use navigational tools, over-reliance on the bridge pilot, the failure of the KyTC and the U.S. Coast Guard to properly maintain or confirm maintenance of the navigational lighting.
